In case of a medical emergency, First Responders will look for this bottle to obtain important medical information that assists in providing you the care that aligns with your condition and your wishes. The bottle and its associated fridge magnet can be obtained free of charge from fire department personnel.

Note that the Medical Orders for Scope of Treatment (MOST) and the No CPR forms must be authorized by your medical practioner to be valid.
